There have been a few questions about band physicals so I thought I would repost the following:
The following is from the May 2008 Band buzz:
Each student who participates in band is required to complete an information and health form and have a doctor administer a physical. These are required to be on file with the office prior to summer band camp beginning. The form can be found online - http://www2.ccisd.net/Libraries/Athletics/09-10_Athletic_Physical.sflb.ashx or is available in the front office of each campus. You can either have your family physician complete the physical or come to the high school on Saturday, May 16 between the hours of 8 am until 12 noon and obtain it there. The cost is $25 and the physicals will be administered at the Krueger Field House. Checks should be made payable to Dr. Kirkwood. If you elect to go your personal physician, do not schedule the appointment until May 1 or after as the physicals expire after 1 year. In addition, if your child requires aphysical for any other activity (sports for example), only one physical needs to be obtained and copies are made for groups concerned. Another alternative (thanks Patty Ewert) - area Walmart Clinics (Friendswood, League City and Kemah all have one) are offering school physicals for $25.00 and no appointment is necessary. They are open until 7:00 PM daily including weekends and there is usually very little wait since the clinics are new.
